Meningitis Immunization Record Requirements

  1. Administered in the last 5 years and no later than 10 days prior to the first day of class.
  2. Required Documentation Criteria (Bacterial Meningitis Vaccine Requirement):

    Documentation must meet one of the following conditions:

    • Include a health practitioner’s signature or official stamp, or
    • Be issued by a state or local health authority

    Additionally, documentation must:

    • Be in English and clearly legible
    • Reflect a vaccine approved by the CDC (e.g.,MCV4 or MPSV4)
    • Note that international vaccines not approved by the CDC will not be accepted
    • Clearly display the administration date(month/day/year format)
    • Include the student’s first and last name
    • Include the student’s date of birth
    • Be uploaded by the student through the myUHV Self Service portal
    • Indicate the vaccine required for college bacterial meningitis compliance

How to Submit Your Bacterial Meningitis Records

Fully admitted students must submit their vaccination documentation through Med+Proctor. All students under the age of 22 are required to register with Med+Proctor to meet the bacterial meningitis requirement.

Please note:

Once you are fully admitted, you will have access to the “Admissions” tile in your myUHV Self Service portal, where you can upload your records to Med+Proctor.

Vaccination Record Submission Deadlines

Students registering for Spring, Summer, or Fall terms are required to upload and submit proof of their bacterial meningitis vaccination through Med+Proctor.

To be eligible to register for classes, the vaccination (or booster dose) must:

  • Have been administered within the last five (5) years, and
  • Be received at least 10 days prior to the first day of the semester

Submission deadlines:

  • Fall 2026: August 14, 2026
  • Spring 2027: January 9, 2027
