Graduation

Appeal to Graduate – Request approval to graduate if you missed the graduation application deadline.

Enrollment & Verification

Enrollment/Anticipated Degree Verification Request Form – Request official documentation of your student record, including proof of enrollment, degree completion, graduation anticipation letter, graduation invitation letter, or university name change letter.

Letter of Non-Attendance – Use this form if you applied to the university but did not enroll in any classes, or if you were admitted and registered and dropped all courses without receiving grades or did not register.

Military Service Credit Request – Request academic credit for eligible military service or training.

Personal Information Updates

Update Personal Information Request Form – Request updates to your personal information, including date of birth, mailing address, email address, or Social Security number (documentation required).

Change of Primary Name – Update your official name on record (requires supporting documentation).

Records Access & Privacy

Authorization to Release Educational Records (FERPA) — Family Educational Rights and Privacy Act (FERPA) require you to complete this form to grant another person (such as a parent, spouse, or sponsor) permission to discuss your university records with staff, including billing, financial aid, and academic information.

Permission to Release Records to Victoria College — Authorize the release of your records to Victoria College.

Request to Withhold Public Information – Prevent your directory information (such as name, major, or enrollment status) from being publicly released.

Other Requests

Reinstatement Form – Request to return to the university after being dropped for non-payment.

Records Appeal Form – Appeal a decision related to your academic records.

Note: This form is not used for 1st or 2nd academic suspension appeals. Students appealing a 1st or 2nd suspension must submit the Academic Appeal form through eForms. Students on a 3rd suspension may use this form to submit an appeal.
